Questions on how the circulator of the petition fills it out

We had 4 questions on how to fill out the petitions from the person who circulates them point of view.

1) What goes in the 1st box on the top front of the initiative which says:

... shall be submitted to the qualified electors of the State of Arizona (county, city or town of _________________________ ) for the approval or rejection ...
 
Arizona goes in this box - the initiative is for the whole state of arizona rather then a county, city or town
  Since both of the RAD initiatives are STATE WIDE initiative and will be voted on by everybody in Arizona, then Arizona goes in that box.

And for that matter EVERY RAD petition will need to have Arizona in that box.

An initiative could be for a single county, city or town and in that case the county, city or town would go in that box.

2) What goes in the 2nd box on the top front of the initiative which says:

I am a qualified elector of the state of Arizona, county of (or city or town and county of, as the case may be) ______________________
 
put the county that the people who signed the petition or initiaitive live in
 

In the box you put the county in which the people who are signing the petition are registered to vote in.

If you are collecting signatures of from people who live in and are registered to vote in Phoenix then Maricopa County would go in this box.

And if somebody from Pinal County or some other county comes along, they can't sign the petition. You have to break out a second petition which has Pinal County in this box for them to sign.

3) What goes in the 1st box on the back of the initiative which says:

State of Arizona)
  )
County of _____________)
  
leave this blank - the person who notarizes the petition or initiative will put their county in the box
  This is the county of the person who is NOTARIZING the petition. This will be filled out by the person who notarized the petition, NOT the person who is circulating the petition.

So we should always leave this box BLANK.

4) What goes in the 3rd box on the back of the initiative which says:

but who is otherwise qualified to register to vote in the county of __________________
 
put the county of the person who circulated the petition or initiative in this box
  The county of the person who is circulating the petition goes in this box.

So in the case of any petition I circulate Maricopa County would go here because I live in Maricopa County and I am the person circulating the petition.
